An expired initial, provisional, professional or master educator license generally returns through a three-year Provisional License request using application 3020.
- 02
A Lifetime License missed-background-check case may be restored after a satisfactory check within 180 days of invalidation. The exact reason and date must be known.

A Lifetime License invalid after five consecutive years without qualifying education employment generally returns through a Tier II Provisional License, not direct restoration.
- 04
Tier I, substitute, disciplinary and uncertain records remain outside this check and need direct DPI guidance.
